SERVICE AGREEMENT & WAIVER

ABOUT US AND SERVICES

Lagoballo (ABN: 73 667 038 960) (we, our or us) provides high quality on-site horse vacation care services. For the purposes of the contract, the term Lagoballo refers to the company. The term “client” refers to the horse owner and/or homeowner who are engaging the services of Lagoballo. The term “horse“ (in both singular and plural forms) means any rabbit under the care of Lagoballo.

ABOUT YOU

When we refer to the word “you” or “I” (I, you or your) in this Horse Care Service Agreement & Waiver (Waiver), we are referring to the person who engages Lagoballo to undertake cervices and whose name and signature appears at the bottom of this form.

WAIVER DETAILS

As the client, I understand that potential harm could occur to my horse(s) while under the care of Lagoballo . I agree to release and hold harmless Lagoballo from all liability including its owner or employees, should my horse(s) become lost, injured, or death should my property suffer any damage, not due to any negligence on Lagoballo's part.

l, hereby grant permission to Lagoballo to act on my behalf, and in my horse's best interest, by obtaining veterinary care, should deem it necessary for the health and well-being of my rabbit(s). I further agree to pay for the cost of any veterinary, or reasonably necessary, services whose costs Lagoballo may incur (for example, any additional hay/feed, should you run out during the contract time period). Further, l attest that my horse(s) are up-to-date on all vaccinations (mainly strangles and tetanus) and will provide proof of it. l also, attest that my horse(s) doesn't/don't suffer from any life threatening or contagious condition that may be exacerbated by exposure to other animals while in Lagoballo's care.

Understanding the risks as stated above, I freely and voluntarily enter into this contract, including the release and waiver, with Lagoballo. This contract is full and complete and hereby agree to these terms.

EFFECT OF THIS DOCUMENT

I have had sufficient opportunity to read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without inducement of any kind.

I understand that my signature to this document constitutes a complete and unconditional release of all liability of Lagoballo, to the greatest extent allowed by law in the event of the horse(s) under care, suffering injury or death, in addition to property damage.
